To complete the notarization, each signer must present one valid, unexpired form of U.S. government-issued ID. Acceptable forms include:

U.S. State ID
U.S. Military ID
U.S. Passport
U.S. Permanent Resident Card (Green Card)

Paper IDs and photographs of IDs are not accepted.
If you do not have a valid U.S. ID, a credible witness may be used (permitted by law). The witness must provide valid identification.
If witnesses are required for your document, I may be able to provide them for an additional fee. Please let me know in advance so arrangements can be made.
Payment is due at the time of service.
